Closure is more than a status label
The Action owner submits real resolution Evidence and a comment. An authorised verifier can approve the result or reject it with a reason, while the history remains intact.

- A Finding carries severity and may come from a Template rule or an authorised manual record.

- An Action has an owner and due date, with rule-based or manual assignment source retained in history.

The sample Operations Standard issue gains accountability and verification within the same record chain.

Fix the Finding
When the Audit completes, automatic and manual Findings remain in the result snapshot and cannot be removed by changing an Answer later.

Severity, owner and due date remain visible together. An authorised administrator can make a recorded override when needed.

Verify the resolution
The owner adds a comment and resolution Evidence. A separate verifier accepts it or rejects it with a reason, preserving the cycle.

About Findings and Actions
Can an Action owner close the work directly?
The owner submits a comment and resolution Evidence for verification. Where verification is required, a separate authorised user makes the final decision.
What happens when the verifier finds the resolution insufficient?
A reasoned rejection is recorded and the Action returns for rework. The earlier resolution and decision remain in history.
